An application for new registration under the Specialist Pathway comprises two stages:
STAGE 1 - TO BE COMPLETED BY THE EMPLOYER
The employer must submit the following documentation to the Medical Board:
- Completed Employer Application Form (Form IMG 1)
- Position Description
- Curriculum Vitae with detailed work history
- Applicant's Statement of Intent to return to country of training on completion of training
- Outcome of College Assessment for a specified training position (Form IMG 8)
- Certified copy of practitioner's original medical degree accompanied by a certified official translation, unless it is in English
- Certified copy of practitioner's original Certificate of Good Standing / Certificate of Registration Status from the authority where the practitioner was last registered
- Certified copy of the original evidence of satisfying the National English Language Proficiency requirement
- Primary Source Verification - evidence of your unique EICS number sourced from the Australian Medical Council
- Supervision Report (Form IMG 6) from the former position if the practitioner is moving from one position to another in NSW
- Training program
- Effective from 1 October 2008, evidence of Professional Indemnity Insurance.
- During this stage, the practitioner must request Certificates of Good Standing / Certificates of Registration Status from all jurisdictions where they have practised or held registration in the last 5 years. These are to be sent directly to the NSW Medical Board from the issuing authority. Certificates must be dated within 3 months as of the date on which the practitioner attends the Board in person.
- Employers must ensure that practitioners hold appropriate Professional Indemnity Insurance (PII), or fit within an exempt category, at the time of registration. Practitioners are required by legislation to provide evidence of PII or exemption to the Medical Board at the time of renewal of registration. Practising without PII or an exemption constitutes unsatisfactory professional conduct.
The Board will advise the employer when Stage 1 is complete so that the application can proceed to Stage 2.
STAGE 2 - TO BE COMPLETED BY THE PRACTITIONER
On completion of Stage 1, the practitioner must submit, in person, the following documents:
- Completed Applicant Registration Form (Form IMG 2)
- Updated Curriculum Vitae since submitting the employer application
- One passport sized photograph
- Two forms of identification, including passport - see National Identification Validation Standard
- Registration and application fees (payable by cash, cheque, money order, MasterCard or Visa).
Applicants must not commence employment before Stage 2 has been completed.
